Artwork preparation
Supply a print-ready PDF at the finished size. Keep your layout inside the page edges and do not include bleed. Identify your ink colors and include a separate back page for two-sided printing. For numbering, specify the starting number, ink color, and placement.

Use a separate cart item for each design. Production scheduling is confirmed after artwork review. Shipping is quoted and invoiced separately after purchase; local pickup is available in Los Angeles. Contact us before ordering if you have a firm deadline or need custom specifications.

Forms use 20 lb NCR paper, print without bleed, and are glued along one edge. Choose Canary / White or Pink / White for 2-part forms.
Optional numbering, hole punching, perforation, and shrink wrapping are priced for the selected quantity. For numbering, choose red or black and enter the starting number and location. Choose hole placement and size if punching is selected. Choose a bundle size for shrink wrapping. Enter perforation placement under Special Instructions.
